Position Summary

Seeking a Transportation Specialist/CNA for our Fargo/Moorhead region. As a transportation specialist this person will be responsible for coordinating and providing transportation to seniors, individuals with disabilities, and hospice patients. Transportation will be to various locations within the Fargo/Moorhead area. You will provide assistance in and out of the vehicle for patients as needed. As a nursing assistant this person would be a vital member of the Interdisciplinary team (IDG). We are seeking a motivated person with initiative to learn, dependable and a team player. This nursing assistant would be responsible for providing personal care services to hospice patients as directed by the patient's plan of care. These services include personal hygiene care, assisting with nutritional needs, providing comfort care and other assistance needed. Safely transferring patients, taking vital signs and complete charting in appropriate time to ensure the team receives complete reporting.

Work within the nursing assistant job scope in order to meet the requirements of applicable federal, state and local regulatory agencies.

KEY RESPONSIBILITIES - Transportation Specialist

  • Transports seniors and individuals with disabilities to and from locations.
  • Coordinates and plans scheduling and routing to and from medical appointments.
  • Routes daily trips in an efficient, effective and safe manner.
  • Resolves scheduling conflicts by staffing with supervisor and/or referring participant to other community resources.
  • Drives vehicle to and from various locations, following a pre-established schedule and adjusting schedule to accommodate occasional schedule additions
  • Maintains driver log; if required, lists client name, address and other information for each trip

KEY RESPONSIBILITIES - Certified Nursing Assistant
  • Provides for patient's personal hygiene and daily living. This may include providing and clearing bedpans and urinals, providing baths, backrubs, shampoos and shaves, and assisting patients with showers. Assists with meals and feeds patients. Ambulates, turns and positions patients. Provides fresh water and nourishment between meals.
  • Administers enemas, douches, nonsterile dressings, surgical preps, ice packs, heat treatments, sitz and therapeutic baths, and applies restraints.
  • Checks vital signs and weight; performs sugar and acetone urine testing, specimen collection, and post-mortem care.
  • Works effectively as a team member in the delivery of care and assists in the orientation of new employees. Maintains good working relations with nursing unit staff and other hospital employees through appropriate communication.
  • Instills confidence in patients by maintaining a neat appearance and performing duties in a competent and caring manner. Reports observations of patients to the nurse caring for the patient to ensure professional assessment.

Qualifications

REQUIRED EDUCATION, CERTIFICATIONS, AND EXPERIENCE
  • Certified Nursing Assistants registration from North Dakota and Minnesota State Department of Health - preferred
  • Valid drivers license and auto insurance - required

PREFERRED QUALIFICATOINS
  • Must be 21 years of age or older
  • Two years' experience driving and transporting patients
  • One year patient care experience in a health care setting; hospice or home health experience preferred
  • High School Diploma/GED
  • Ability to be flexible and effectively manage priorities, multiple tasks and meet deadlines
  • Ability to work with and communicate effectively with terminally ill patients and their families
