Join Our Team as a Care Manager!We are currently seeking a dedicated and compassionate Care Manager to join our Developmental Disabilities Division at All Embracing Home Care in Fargo, ND. The ideal candidate will have a passion for helping individuals with developmental disabilities lead meaningful and fulfilling lives.Key Responsibilities:Coordinate care services for clients with developmental disabilitiesDevelop care plans tailored to each client's unique needs and goalsCollaborate with clients, their families, and a multidisciplinary team to ensure high-quality careMonitor and evaluate client progress, adjusting care plans as neededProvide support and guidance to direct care staffAdvocate for clients' rights and access to servicesEnsure compliance with regulatory standards and company policiesQualifications:Bachelor's degree in social work, psychology, or related fieldExperience working with individuals with developmental disabilitiesStrong communication and interpersonal skillsAbility to work independently and as part of a teamEmpathy, patience, and a positive attitudeValid driver's license and reliable transportationWorking Hours:This full-time position offers a Monday to Friday schedule from 8:00 AM to 5:30 PM, providing a healthy work-life balance for our team members.About Us:All Embracing Home Care is a leading provider of home care services in the Fargo, ND area.
