We offer benefits packages for health, dental, and vision insurance as well as PTO and paid holidays.Applicants must:Be 18 years of age or olderHave one year of Direct Care experienceHave a Bachelor's degree (B.A., B.S.) in a human services related area or related field and be able to provide evidence of that degreeHave one year of direct care experience (equivalent to 2080 hours)Pass a background studyHave a current driver's license, auto insurance, and an operating vehicleEmployment is contingent upon providing agency evidence of degree in related area as well as clear background checks.Employment is contingent upon providing agency evidence of degree in related area as well as clear background checks.JOB RESPONSIBILITIESEvaluations:Completing direct report performance reviewsConducting site reviews as directed by the Program ManagerComplying with all agency policies regarding EEO/AA and HIPAASupervisory and Administrative Responsibilities:Performing all of the responsibilities of theDirect Care Professional position as is necessary (including direct client supervision)Directly supervising Direct Care Professionals including, but not limited to: directing work, conducting performance reviews, rewarding and disciplining employees, addressing complaints, and resolving conflicts.Accurate and timely response to information requests from outside agencies or individuals as appropriateAttendance and participation in staff meetingsRegularly reporting to the Program Manager as to the current status of each client.Direct Care Professional scheduling, monitoring of time and attendance, etc.Other duties, as assignedProgram Responsibilities:Positive interactions with clients, guardians, clientfamilies, representatives of other agencies, and other persons with whom clients interactAdmission and orientation of new clientsDevelopment and maintenance of the client paperwork including, but not limited to: goal/objective writing, observation of program implementation, ensuring proper documentation, and service recommendationsConducting and coordinating client reviews, team meetings, and site meetings, as is necessary or determined by the Program ManagerPreparation of client transfer and discharge summariesCompliance with state and local licensing regulationsEffective implementation of behavioral plans, if applicableProperly acting upon, documenting, and distributing Incident ReportsOther duties, as assignedTeam:Supports and participates in common teamwork; cooperates and works together with supervisors and co-workersUses tactful, appropriate and professional communicationFollows up with supervisor as appropriate regarding concerns and complaintsPromotes positive relations among agency employeesMaintains confidentiality relating to all agency related data and mattersShall be capable of relating to individuals at various levels; as unique situations present themselves, the incumbent must be sensitive to agency needs, employee goodwill and public imageMedical and Health Responsibilities:Oversight, coordination, and proper documentation of all client health, medical, and psychiatric careCoordination of referrals to consultantsOther duties, as assignedFinancial Responsibilities:Maintaining accurate financial records for each client on a monthly basis, if applicableOther duties, as assignedOn-Call Responsibilities:Will be compensated for performing On-Call duties when assignedTraining Responsibilities:Case Manager shall annually complete required training.Start your fun and rewarding career at Access of the Red River Valley today!EEO/AA EmployerCompensation $28.20-$39.74 per hour
