Are you a communicator who thrives on helping others navigate the healthcare system? OUr client is looking for a Medicaid Care Management Coordinator to join their team. In this role, you will be the primary point of contact for Medicaid members, specifically focused on offering their Disease Management Health Coaching program.
You'll serve as the "front-line" support, bridging the gap between members, providers, and community resources. If you are comfortable on the phone, tech-savvy, and passionate about improving health outcomes through proactive outreach, this is the role for you.

Responsibilities
- Perform outbound calls to eligible Medicaid members to explain and offer the Disease Management Health Coaching program.
- Act as the first point of contact for member and provider inquiries regarding care plan procedures, protocols, and scheduling.
- Connect members to health plan benefits and local community resources to address Social Determinants of Health (SDOH).
- Support member onboarding by sending welcome letters, program materials, and educational correspondence.
- Document all non-clinical member interactions in accordance with state and regulatory guidelines to ensure a seamless record for providers and staff.
- Work independently to resolve member or provider concerns, ensuring high-quality customer service at every touchpoint.
