Social Services is now accepting applications to be considered for a Full-Time Social Worker in the Home and Community-Based Services Unit.

Salary Range: $33.36-$47.06 per hour

This position will be eligible for Clay County's benefit package.

Minimum Requirements:

  • Bachelor's degree from an accredited four-year college or university with a major in Social Work, Psychology, Sociology or closely related field.
  • 1-3 years prior work-related experience
  • Valid driver's license
  • LSW Preferred
  • Successfully complete a background investigation

Duties and Essential Functions:

  1. Provide case management and support coordination for individuals with developmental disabilities or related conditions under Rule 185 and home- and community-based service programs.
  2. Assess individual and family needs, develop and implement service plans, and coordinate services to promote independence and community living.
  3. Collaborate with interdisciplinary teams, including health care providers, schools, and community resources, to ensure integrated and person-centered care.
  4. Monitor and evaluate ongoing service effectiveness and client progress, ensuring compliance with state and federal program requirements.
  5. Maintain accurate and timely documentation in accordance with agency and state guidelines.
  6. Participate in multidisciplinary team meetings and county/state trainings as required.
  7. Other duties as assigned.
