Job Type: 2-Month Contract (with possibility of extension based on business needs)
We're looking for a compassionate and detail-oriented Patient Care Coordinator to support patients throughout their care journey. This role serves as a centralpoint of coordination between patients, providers, facilities, pharmacies, and insurance partners to ensure timely, accurate, and well-communicated care.

Key Responsibilities
Serve as the primary point of contact for patient questions related to referrals, imaging, lab work, medications, and treatment plans
Collaborate with healthcare providers, clinics, and external facilities to coordinate patient care services
Manage referrals, tests, procedures, and follow-ups to ensure care is delivered efficiently and on time
Coordinate medication and authorization information with pharmacies and insurance providers
Communicate updates to patients and healthcare facilities regarding lab results, imaging status, and service timelines
Schedule appointments and manage scheduling requests using appropriate resources for all appointment types
Apply critical thinking to identify issues, evaluate options, and implement effective solutions
Maintain accurate and up-to-date patient records, demographics, and documentation to support continuity of care and compliance
Monitor patient progress and satisfaction to identify gaps or opportunities for improvement
Communicate with empathy, professionalism, and clarity across phone and written channels
Perform additional duties as assigned to support care coordination needs
Qualifications
Required
High school diploma or GED
Proficiency with Google Workspace, Microsoft Office, and standard computer systems
Strong organizational, communication, and documentation skills
Ability to work independently in a remote, high-volume environment
Preferred
2+ years of experience in healthcare or customer service settings
1+ year of experience working with EHR systems (athenaOne preferred)
Understanding of medical terminology and healthcare insurance processes
Prior remote experience handling inbound and outbound calls

Pay and Benefits
The pay range for this position is $23.00 - $23.00/hr.
